    Social Service Assistant
    Part-Time (Tues., Wed., Thurs.)
    To assist in providing social services and ensures quality care to LTCC/Alzheimer's residents and swing bed patients at the hospital

    ESSENTIAL DUTIES & RESPONSIBILITIES:
    Assists in all areas of the office: charting, care conference scheduling, phone and written communication of resident/patient families, shopping for residents, reading and visiting with residents/patients, transports residents to the doctor and/or dentist appointments. Assists residents/patients with advanced directives. Attend educational and other mandatory meetings as required. Regular attendance and punctuality. Other duties as assigned.

    MINIMUM QUALIFICATIONS:
    Current C.N.A. license in good standing or obtain within 6mo. CPR certificate. Basic knowledge of medical terms and medical concerns. Excellent interpersonal and communication skills. Good computer skills. Must have a valid driver's license. Must obtain Social Service Certificate with one year of hire.
